On a stormy night in 2014, Detective Sebastian Castellanos, a man broken by the loss of his family in a fire years ago, answers an emergency call at Krimson City's run-down Beacon Memorial Hospital. Together with his companions Joseph Oda and Juli Kidman, he arrives at the place where multiple brutal homicides are reported. What they find is a slaughterhouse: mutilated bodies, blood everywhere and a single babbling survivor, Dr. Marcelo Jiménez. As they review security footage, a hooded, disfigured figure — later known as Ruvik — appears in the footage and, suddenly, in reality. A prick in the back of the neck, a white flash... and Sebastian wakes up in a distorted world, hanging by his ankles in an underground morgue infested with horrors.
